Identification & Proof of Residence
To get a Sayville Library card you will need one (1) form of identification. A listing of acceptable forms of identification are as follows:
- A valid NYS driver license
- A current Town of Islip ID card
- A utility bill, dated within 30 days, showing residence within the Sayville School District boundaries
- A copy of a lease agreement, mortgage or tax bill showing residence within the Sayville School District boundaries
- A current Sayville School ID (for students)
- A current College or University ID (for students)
To receive a child or teen library card, the parent or guardian must be present to sign the registration card for anyone under 18. DVDs may be checked out on these cards if permission is given by the parent/guardian by signing the back of the registration card. With parental permission, children and teens will be able to access the internet from the library.
Library cards expire every three (3) years. A valid driver license or a piece of mail, stamped within the last 30 days, are acceptable forms of identification for a library card renewal.
